Fitness Center Options

Employees and retirees can take advantage of using the following 15 Park and Recreation Fitness Centers for a flat fee of $75 per member, per year.

To participate in the fitness center options:

1. Complete a “membership enrollment form” (Word document). You may elect to have two consecutive payments of $37.50 deducted from your payroll check, or submit a check or money order in the amount of $75.00 payable to the Park and Recreation Department.


2. If you are enrolled in the City’s health plan, please register on myuhc.com and complete a health risk assessment on the website. Please print the confirmation statement.


3. For employees not participating in the City’s health plan, register on www.healthatoz.com and complete the health risk assessment on the website. Please print the confirmation statement.


4. Please bring the following documents to 6AS to obtain your membership:
   
   
   1. Membership enrollment form
   2. Confirmation statement from the Health Risk Assessment
   3. Employee Identification Badge
   
   
5. Employee Benefits will send the enrollment information to the Park and Recreation Department to validate your membership. A membership ID card will be issued by the fitness center in your community.
